PENGGUNAAN FORMAT ISBAR3 BERBASIS ELEKTRONIK DALAM HANDOVER KEPERAWATAN UNTUK MENINGKATKAN KOMUNIKASI EFEKTIF

Serah terima (handover) keperawatan merupakan bagian penting dari perawatan klinis di mana informasi pasien yang relevan perlu dikomunikasikan secara akurat dan tepat, untuk memastikan kesinambungan perawatan dan keselamatan pasien. Komunikasi yang buruk dan format yang tidak terstruktur merupakan faktor yang dapat menyebabkan pasien berisiko dalam perawatan. Tujuan penulisan untuk menjelaskan format terstruktur yang terkomputerisasi yang digunakan oleh perawat dalam serah terima keperawatan dengan menggunakan metode komunikasi efektif: Identify, Situation, Background, Assessment, dan Rekomendation, Read-Back, Risk (ISBAR₃). Metode analisis dengan kajian literatur. Hasil kajian didapatkan bahwa penggunaan format ISBAR₃ berbasis elektronik terstruktur ini digunakan untuk mendukung cara serah terima verbal melalui model komunikasi terkini yang lebih sistematis, tepat, akurat, dan relevan dalam waktu yang sangat singkat. Penggunaan format terstruktur berbasis elektronik ini merupakan bagian dari sistem informasi keperawatan di rumah sakit. Keberhasilan penerapan format serah terima elektronik ini dapat meningkatkan komunikasi efektif, dan meningkatkan  keselamatan pasien.

ABSTRACT

Nursing handover is a vital part of clinical care where relevant patient information needs to be communicated precisely and accurately, to ensure continuity of care and patient safety. Poor communication and lack of structured format are contributing factors in adverse incidents where patient care is put at risk. This study aims to explain the computerized structured format used by nurses in the handover of nursing using effective communication methods: Identify, Situation, Background, Assessment, and Recommendation, Read-Back, Risk (ISBAR3). Method of analysis with literature review. Using this ISBAR3 computerized structured format is part of the hospital's nursing informatics system. The results of this study obtained that structured electronic format was used to support the verbal handover report through more systematic, precise, accurate, and relevant model of communication in very short period of time. The successful implementation of this electronic handover format can improve effective communication and patient safety.
