Sebuah studi yang membandingkan penggunaan asam kojic 4% dengan krim mulberry untuk pengobatan melasma
Abstract
Abstrak. Resisten terhadap terapi dapat dilihat pada pengobatan melasma, gangguan hiperpigmentasi kulit yang biasa ditemukan. Krim asam kojic dan mulberry telah dilaporkan berguna untuk pasien dengan melasma, namun ada beberapa uji coba terkontrol yang membandingkan kedua regimen ini untuk melasma. Untuk membandingkan efektivitas krim asam kojic 4% dan mulberry dalam pengobatan melasma selama 8 minggu pemberian. Penelitian ini adalah penelitian eksperimental, studi single-blinded. Pasien terdaftar dari pasien baru yang datang ke Poliklinik Dermatologi di Rumah Sakit Dr.M.Djamil Padang untuk mengobati melasma mereka dari Februari 2017 hingga April 2017. Penilaian dilakukan dengan membandingkan sisi kanan dan kiri wajah (studi perbandingan kanan-kiri) secara berurutan. Pemberian krim asam kojic 4% dioleskan ke pipi kanan, sedangkan krim mulberry dioleskan ke pipi kiri. Perawatan diberikan sebagai krim malam, dan semua pasien diberikan tabir surya dengan SPF 30 untuk melindungi kulit mereka di pagi dan sore hari. Dua puluh pasien yang didiagnosis melasma dimasukkan. Usia dari peserta berkisar antara 30 hingga 59 tahun. Lima belas pasien (37.5%) memiliki riwayat faktor pemicu kosmetik. Sebagian besar pasien (90%) menunjukkan melasma tipe epidermal dan didistribusikan sebagai pola centrofacial (50%). Setelah 2 bulan terapi, terdapat perbaikan yang signifikan dari melasma yang diobati dengan krim asam kojic 4% dengan nilai p = 0.045 dibandingkan dengan pemberian krim mulberry. Efek samping dari krim asam kojic 4% antara lain rasa terbakar dan kemerahan pada wajah 3 hari pertama. Krim mulberry juga menyebabkan sensasi terbakar dalam terapi awal dan menghilang setelah satu minggu terapi. Pada akhir penelitian, penggunaan krim asam kojic memberikan hasil yang lebih baik daripada krim mulberry.

Abstract.Resistance to therapy is seen in the treatment of melasma, a common skin hyperpigmentation disorder. Kojic acid cream and mulberry cream have been reported to be useful in patients with melasma, but there have been few controlled trials comparing these two regimens for melasma. To compare the effectiveness of 4% kojic acid cream and mulberry in the treatment of melasma for 8 weeks. This research is an experimental research, single-blinded study. Patients were enrolled from new patients who came to the Dermatology Polyclinic at Dr.M.Djamil Hospital Padang to treat their melasma from February 2017 to April 2017. The assessment was carried out by comparing the right and left sides of the face (right-left comparison study) sequentially. Giving 4% kojic acid cream is applied to the right cheek, while mulberry cream is applied to the left cheek. The treatment is given as a night cream, and all patients are given a sunscreen with an SPF of 30 to protect their skin in the morning and evening. Twenty patients diagnosed with melasma were included. The ages of the participants ranged from 30 to 59 years. Fifteen patients (37.5%) had a history of cosmetic precipitating factors. Most of the patients (90%) showed melasma of the epidermal type and distributed in a centrofacial pattern (50%). After 2 months of therapy, there was a significant improvement in melasma treated with 4% kojic acid cream with p = 0.045 compared to mulberry cream. Side effects of 4% kojic acid cream include burning and redness of the face for the first 3 days. Mulberry cream also caused a burning sensation in the initial treatment and disappeared after one week of therapy. At the end of the study, using kojic acid cream gave better results than mulberry cream.
