Karakteristik pattern histopatologi anaplastic thyroid carcinoma di Rumah Sakit Umum Pusat H. Adam Malik Medan periode 2016-April 2021

Abstract.

Background: Thyroid carcinoma is the most common malignant endocrine neoplasia. Anaplastic thyroid carcinoma (ATC) accounts for about 1% of all thyroid neoplasia and can arise de novo or by dedifferentiation from differentiated tumors (PTC and FTC).

Objective: To determine the characteristics of patients with anaplastic thyroid carcinoma at H. Adam Malik Hospital in 2016 – April 2021.

Research Methods: This study is a descriptive study with a cross-sectional approach. All clinical and pathological characteristics were obtained through medical records and pathology archives.

Results: The number of samples of anaplastic thyroid carcinoma found was 21 samples. The mean age of the patients was 63 years, with the age group > 55 years, 71.4% were found. Female patients were found in 57.1%. For tumor size, T4 was found as much as 71.4%, while the absence of lymph nodes (N0) was found in 52.4%, and the absence of distant metastases (M0) was 81%, and the presence of distant metastases (M1) as much as 19%. Patients with stage IVB were found in 66.7%, and the histological pattern of sarcomatoid was found in 48%, while the giant cell pattern was 19%.

Conclusion: Anaplastic thyroid carcinoma in this study was found mostly in older age, and slightly dominated by women. Distant metastases were found in the brain and lungs, and the histologic pattern of giant cells was recently reported by a study that showed a more aggressive clinical course.
