Hubungan kejadian drug related problems (DRPs) penggunaan valproat terhadap kontrol kejang pada pasien epilepsi anak di poli anak Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh

Abstract


ABSTRAK

Epilepsi adalah kondisi kejang berulang tanpa provokasi yang terjadi pada 2 kejadian kejang atau lebih. Prevalensi epilepsi pada bayi dan anak-anak cukup tinggi, menurun pada umur dewasa muda dan pertengahan, kemudian prevalensinya meningkat pada umur lanjut usia. Penelitian ini bertujuan untuk untuk mengetahui hubungan kejadian Drug Related Problems (DRPs) penggunaan valproat terhadap kontrol kejang pada pasien epilepsi anak di Poliklinik Anak RSUD dr. Zainoel Abidin Banda Aceh. Penelitian ini berjenis observasional yang bersifat analitik dengan metode cross sectional. Alat ukur yang digunakan dalam penelitian ini dalah kuesioner Adherence to Medication and Clinic Appointments (AMCA) dan Kuesioner Hague Seizure Severity Scale (HASSS). Penentuan jumlah sampel menggunakan teknik purposive sampling dengan rumus Lemeshow didapat jumlah sampel penelitian sebanyak 68 orang. Hasil penelitian ini menunjukan bahwa presentase paling besar terdapat dalam kelompok usia masa remaja awal (12-16 tahun) sebanyak 29 pasien (42,6%) dan mayoritas pasien adalah perempuan yaitu sebanyak 36 pasien (52,9%), jumlah pasien yang mengalami kejang umum/general yaitu 44 pasien (64,7%) , Sodium Valproat merupakan obat yang paling banyak digunakan pada pasien epilepsi anak yaitu sebanyak 43 pasien (63,2%), piracetam merupakan oba tambahan paling banyak digunakan yaitu 26 pasien (38,2%). Kelompok durasi terapi paling banyak yaitu durasi terapi selama 4-7 tahun yaitu 33 pasien (48,5%)  dengan usia awitan paling tinggi pada kelompok usia awitan 4-7 tahun yaitu 30 pasien (44,1%). Terdapat 64 pasien (94,11%) yang mendapatkan dosis yang tepat. Berdasarkan evaluasi kepatuhan pengobatan yang ditanyakan kepada orang tua atau wali pasien, 56 pasien (82,3%) patuh dalam menjalani pengobatan anaknya. Pasien yang mengalami kejang lebih dari 1 kali selama 3 bulan terakhir sebanyak 35 pasien (51,5%). Berdasarkan evaluasi kuesioner HASS, pasien yang masuk kedalam kategori kejang yang parah sebanyak 38 pasien (55,8%). Terdapat hubungan ( p-value 0,029 < 0,05) antara kejadian Drug Related Poblems (DRPs) terhadap kontrol kejang pada pasien epilepsi

ABSTRACT

Epilepsy is a condition of recurrent seizures without provocation that occurs in 2 or more seizure events. The prevalence of epilepsy in infants and children is quite high, decreasing in young and middle adulthood, then the prevalence increases in the elderly. This study aims to see the relationship between the Drug Related Problems (DRPs) use of valproate on seizure control in pediatric epilepsy patients at the Children's Polyclinic Hospital of dr. Zainoel Abidin Banda Aceh. This research is an observational type of research which is analytic with cross sectional method. The measuring instruments used in this study were the Adherence to Medication and Clinic Appointment (AMCA) questionnaire and the Hague Seizure Severity Scale (HASSS) questionnaire. Determination of the number of samples using purposive sampling technique with the Lemeshow formula obtained a total sample of 68 people. The results of this study indicate that the largest presentation was in the adolescent age group (12-16 years) as many as 29 patients (42.6%), the incidence of epilepsy in pediatric occurred in female patients as many as 36 patients (52.9%). 44 patients (64.7%) experienced generalized seizure, sodium valproate was the most widely used drug in pediatric 43 patients (63.2%) and the most widely used additional drug was piracetam 26 patients (38.2%). The most therapy duration group was the duration of therapy for 4-7 years, namely 33 patients (48.5%) with the highest onset age in the onset age group of 4-7 years, namely 30 patients (44.1%). There were 64 patients (94.11%) who received the correct dose. Based on the treatment evaluation that was asked of the patient's parents or guardians, 56 patients (82,3%) were obedient in undergoing treatment for their children. There were 35 patients who had seizures more than once in the last 3 months (51.5%). Based on the evaluation of the HASS questionnaire, there were 38 patients (55,8%) who entered the severe seizure category. There was a relationship (p-value 0.029 <0.05) between the incidence of Drug Related Poblems (DRPs) on seizure control in pediatric epilepsy patients.
