Pemfigus Vulgaris: Laporan Kasus (Pemphigus vulgaris after covid-19 vaccine: association or coincidence?)
Abstract
karena produksi autoantibodi terhadap protein desmosomal kulit, yaitu desmoglein (Dsg) 1 dan desmoglein 1 Dsg3. Beberapa faktor termasuk kerentanan genetik, obat-obatan tertentu dan keganasan telah dilaporkan memicu atau memperburuk PV. Tujuan: Melaporkan kasus PV yang berhubungan dengan vaksinasi COVDI-19. Kasus: Seorang pria berusia 41 tahun dirawat dengan keluhan utama kulit melepuh di sekujur tubuhnya sejak Juli 2021, 2 hari setelah menerima dosis pertama vaksin Sinovac Covid-19. Pemeriksaan histopatologi menunjukkan bula suprabasal dengan akantolisis dimana lapisan antara stratum basal epidermis dan sisa epidermis tampak longgar dan tidak utuh dengan gambaran “Row of Tombstones”. Diskusi: Efek imunogenik vaksin menyebabkan perubahan kadar kemokin dan sitokin, yang mengaktifkan sistem imun bawaan dan adaptif. Reaksi inflamasi terpolarisasi Th2 diperkirakan berperan dalam kasus ini. Kesimpulan: Berdasarkan anamnesis, pemeriksaan fisik, dan pemeriksaan histopatologi, ditegakkan kasus PV pada seorang laki-laki berusia 41 tahun. Pengobatan dengan injeksi dan steroid oral, imunosupresan oral dapat memberikan hasil yang memuaskan.
