Psoriasis gutata pada laki-laki dewasa: sebuah kasus jarang (Adult males with guttate psoriasis: a rare case)

Abstract


ABSTRAK 

Latar Belakang: Psoriasis gutata ditandai dengan bentuk papul atau plak seperti “tetesan” pada batang tubuh dan proksimal ekstremitas serta umumnya terjadi pada anak-anak dan remaja.

Ilustrasi Kasus: Laki-laki, 40 tahun, datang dengan keluhan gatal di seluruh tubuh sejak enam bulan. Keluhan ini diawali dengan riwayat batuk-pilek yang berulang disertai dengan nyeri ketika menelan. Status dermatologis dijumpai pada regio thorakalis anterior et posterior, abdomen, palmar dextra et sinistra, pedis dextra et sinistra, tampak lesi papul dan plak eritema dan hiperpigmentasi, batas tegas, tepi reguler, disertai dengan skuama tipis, multipel, ukuran gutata, diseminata, konfigurasi tidak khas, dan generalisata. Kaarsvlek phenomenon didapatkan hasil positif. Pemeriksaan histopatologi didapatkan akantosis minimal, rete ridge memanjang, munro microabscess, dan kogoj microabscess. Pasien didiagnosa dengan psoriasis gutata. Terapi yang diberikan berupa terapi topikal kombinasi.

Simpulan: Psoriasis gutata merupakan salah satu jenis psoriasis dan jarang terjadi. Psoriasis gutata didahului oleh infeksi streptokokus yang dapat terjadi pada saluran napas atas. Pengobatan psoriasis gutata dengan pemberian asam salisilat, kortikosteroid topikal, pelembab, dan preparat tar.

ABSTRACT

Background: Children and adolescents are frequently affected with guttate psoriasis, which is characterized by papules or plaque-like droplets on the trunk and proximal extremities.

Case Ilustration A 40-year-old male arrived complaining of itching all over his body for the past six months. A history of repeated colds and swallowing discomfort led to the onset of this issue. Dermatological status was discovered in the thoracic region anterior and posterior, abdomen, palmar dextra et sinistra, and pedis dextra et sinistra. Erythematous and hyperpigmented papules and plaques were seen, along with thin, multiple scales, guttate size, disseminated, configuration atypical, and generalized. Positive results for the Kaarsvlek phenomena were obtained. Histopathological examination showed minimal acanthosis, elongated rete ridge, munro microabscess, and kogoj microabscess. Patient was identified as having guttate psoriasis. The therapy given is a combination of topical therapy.

Conclusion: One uncommon kind of psoriasis is called guttate psoriasis. A streptococcal infection that can happen in the upper respiratory tract precedes guttate psoriasis. Salicylic acid, topical corticosteroids, moisturizers, and tar formulations are used to treat guttate psoriasis.
