Fistula Perilimfatik: Pendekatan Biomarker Cochlin Tomoprotein (CTP) sebagai Peluang Modalitas Diagnostik

Abstract


Abstrak. Fistula perilimfatik (PLF) adalah kondisi dimana terdapat hubungan abnormal antara ruang telinga tengah dan kompartemen perilimfatik di telinga bagian dalam. Hubungan ini bisa disebabkan oleh adanya trauma, spontaneus, kongenital, pasca bedah atau akibat penyakit otologik seperti kolesteatoma. Diagnosis PLF setelah operasi relatif mudah, namun saat pasien datang dengan manifestasi klinis klasik akan membutuhkan pemeriksaan yang lengkap agar mampu menyingkirkan penyebab lainnya. Kebanyakan metode yang digunakan untuk mengidentifikasi PLF kurang sensitif dan spesifitas diagnosisnya tidak memberikan hasil yang konsisten. Kemajuan teknik pencitraan dan teknologi baru dengan pendekatan biomarker saat ini telah menjadi peluang modalitas yang baik dalam mendefinisikan dan mendiagnosis PLF, meskipun anamnesis dan pemeriksaan fisik yang tepat adalah elemen kunci untuk pemeriksaan pasien dengan fistula perilimfatik.

Abstract. Perilymph fistula (PLF) is a condition where there is an abnormal connection between the middle ear space and the perilymphatic compartment in the inner ear. This relationship caused by trauma, spontaneous, congenital, post-surgery or due to otologic disease such as cholesteatoma. Diagnosis of PLF after surgery is relatively easy, however when a patient presents with classic clinical manifestations, a whole examination is required to rule out other causes. Most of the methods used to identify PLF were lack of sensitivity and the diagnostic specificity were inconsistent. Advanced imaging techniques and new technologies approache biomarker have now emerge as an excellent modality and opportunity in defining and diagnosing PLF, despite the fact that proper anamnesis and physical examination are the key elements in the examination of patients with perilymph fistula.
